University of Virginia Registered Nurse (RN) Case Manager Inpatient in Charlottesville, Virginia

$10,000 Sign on Bonus available for qualified candidates

Within the inpatient setting to assess and address patient and family needs related to effective and efficient movement through the hospital system and beyond. Perform initial assessment on all patients and begin discharge process from day one of patient’s stay. Complete level of care assessment to ensure patients are receiving the appropriate level of care, and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Maintain expert knowledge of Interqual® Level of Care criteria. Work closely with the Admitting Physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinate discharges home and facilitate transfers to external placement facilities

  • We are seeking a skilled clinician to utilize nursing knowledge and experience to join the UVA Case Management team to assess and address patient and family needs related to effective and efficient patient progression through the hospital system and beyond. Perform initial assessment on all patients and begin discharge process from day one of patient’s stay. Complete level of care assessment to ensure patients are receiving the appropriate level of care, and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Work closely with the Admitting Physicians and the care team to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinate discharges home and facilitate transfers to post-acute care facilities to ensure safe transition to the next level of care.

  • In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range: $81,328.00 - $108,035.20 Annual

MINIMUM REQUIREMENTS

Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.

Experience: 3 years Relevant Experience

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. Accredited Case Manager (ACM) Certification within 3 years of hire.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
