Sevita Transportation Driver Program Assistant in Brockton, Massachusetts
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. SUMMARY
The Driver and Program Assistant, Transportation provides safe, reliable transportation services between clients’ homes, day centers and to/from appointments as needed; and coordinates with the transportation supervisor and clinical team. The Driver and PA drives clients and staff to events and outings as directed, and offers safety assistance when boarding and off-loading at all destinations. In the capacity as a Program Assistant, additional responsibilities include participation at the day center to facilitate activities, meals and care as needed.
Monday - Friday 7am - 3:30pm - No Weekends!
Pay Rate: $18 Hourly
*Must be bilingual Haitian Creole
*Must have a MA drivers license for 1 year
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
Transportation
Provides safe transportation of clients between home and day center, as well as to/from appointments and program activity locations
Operates the vehicle in a defensive, safe and courteous manner at all times, including in adverse weather conditions
Obeys all speed limits and local and state traffic laws
Operates a two-way radio to maintain communication with dispatcher
Notifies dispatcher if a member if not home upon arrival (or pick-up) and if a member cannot be returned home due to absence of required supervision
Maintains daily time log
Assists adult day services members from door to vehicle, when necessary, and in and out of the vehicle at all times
Ensures passengers are wearing seat belts whenever the vehicle is in motion
Lifts and transfers members as needed using proper body mechanics and assistance
Operates a wheelchair lift, when necessary
Responsible for the vehicle maintenance
Conducts daily vehicle inspection using cheek list and informs supervisor of any problems
Maintains vehicle interior
Ensures the vehicle is serviced per scheduled maintenance plan
Notifies supervisor in the event vehicle requires emergency service or repairs
Participant’s Care and Safety
Assists participants in with activities of daily living, including basic personal care, and attending to medical appointments, etc.
Assists in monitoring clients during breaks and lunchtime, including behavioral concerns, bathroom assistance and general assistance as needed.
Manages physical transfer of participants by following training guidelines
Responsible for participant safety and well-being at all times
Able to recognize signs and symptoms of abuse/neglect and notifies the supervisor Reports to supervisors for any unusual physical or behavioral changes
Understands "Client Rights and Responsibilities" and serves as an advocate for the Participant
Interacts with participants in an empathetic, respectful, caring, and dignified manner
Programs, Recreation and Activities
Assists in the implementation of social, recreational and educational programs, including encouraging social interactions among members
Assists participants through participation in activities
Accompanies participants in scheduled activities, appointments or others as designated
Assists other staff in carrying out program assignments
Other Duties
Completes necessary documentation, reports, etc. in a timely, thorough, and accurate manner
Attends staff meetings, conferences, in-services training as assigned by supervisor
Successfully completes mandatory training
Assists with compliance of policies and procedures, regulatory and legal requirements of practice
Follows confidentiality requirements at all times
Adheres to Participant Bill of Rights and other policies and procedures
Performs other related duties and activities as required.
SUPERVISORY RESPONSIBILITIES
None required.
Minimum Knowledge and Skills required by the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
Education and Experience:
High school diploma or its equivalent and at least 18 years of age
Prior van driving experience preferred
Experience in transporting seniors or disabled persons preferred
Minimum one year experience in providing services to a special needs population, preferably with the elderly
Certificates, Licenses, and Registrations:
Current valid Driver’s License and car insurance required
Class B Commercial Driver’s License (CDL) if required by state
All state required training and certification completed in mandated timeframes

Clean Driving record
Physical in the past 12 months
Annual TB test
Competence in CPR and First Aid, as required by state regulations
Ability to read and respond to written job instructions
Ability to read road maps and street signs
Physical Requirements:
• Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
